The Endocannabinoid System and Inflammation
The endocannabinoid system is deeply embedded in immune function and inflammatory regulation. CB2 receptors are highly expressed on immune cells including macrophages, T cells, and B cells. When activated, CB2 signalling generally suppresses pro-inflammatory cytokine production and promotes resolution of inflammation. This is why cannabis-derived compounds show anti-inflammatory effects across such a wide range of conditions — the target receptor is present throughout the immune system.
CBD's Anti-Inflammatory Mechanisms
CBD inhibits inflammation through multiple pathways: it activates CB2 receptors to suppress inflammatory cytokines (TNF-α, IL-1β, IL-6), inhibits COX-2 enzymes (similar mechanism to ibuprofen), activates TRPV1 receptors which modulate inflammatory pain, and activates PPARγ receptors which have broad anti-inflammatory effects. CBD also reduces oxidative stress, a key driver of chronic inflammation. Rheumatoid arthritis: Several clinical trials show CBD reduces joint pain, stiffness and sleep disturbance. Crohn's disease and IBD: Cannabis has shown significant remission rates in clinical trials; it is prescribed for IBD in several countries including Germany. Multiple sclerosis: Sativex (THC:CBD) is approved in many countries specifically for MS-related spasticity, which involves neuroinflammation. Fibromyalgia: Observational studies consistently report improved symptoms; clinical trials are ongoing. Psoriasis: CBD applied topically shows promise for reducing skin inflammation.
Practical Anti-Inflammatory Use
For general inflammation and inflammatory pain, a high-CBD preparation (10:1 CBD:THC or pure CBD) is the most appropriate starting point. Daily oral dosing (15–50mg CBD) maintains consistent anti-inflammatory blood levels. Topical CBD products are relevant for localised inflammatory conditions like arthritis in specific joints or psoriasis. For severe inflammatory conditions, consult a medical specialist — cannabis-based medicines require careful integration with existing treatments.
